Admission Details
The following credentials are to be submitted upon registration- Form 138 or high school report card
- Certificate of Good Moral Character duly signed by the School Principal
- Four (4) copies of color ID Pictures ("1.5x1.5")
- Certified true copy of birth certificate from the National Statistics Office.
For Transferees
- Certificate of Honorable Dismissal
- Certified Copy of Subjects taken
- Certification of Good Moral Character
- Photocopy of Birth Certificate (NSO)
- Four (4) copies of ID Pictures (1.5"x1.5")
